Provide some basic enrollment and claims information through the Group Evaluation Form to complete Phase 1. You will receive a Scorecard with key indicators of fit within 48 hours.

Request a Quote
If Scorecard insights and consultant feedback qualify the group as a fit, you will be invited to proceed to Phase 2 and receive a proposal.
